Media Consent form for Company Events
Tata Consultancy Services Switzerland Ltd., Zurich (“TCS”, “we” or the "Company"), and / or sometimes through its contracted partners will be occasionally recording events (the "Event “) organized by the Company as the data controller. This consent will serve as your media release concerning your participation at the TCS and Google Cloud roadshow: Reimagining business with the power of AI and cloud, Zurich, Switzerland.
- With the participation to the event and the acceptance of this consent, you grant TCS, its affiliates and other entities that TCS may designate from time to time, the right to record, stream, film and photograph your presence and activities at the Event (the "Testimonials") and to distribute, broadcast, stream, edit, translate or otherwise disseminate it, without any further approval from you, in whole or in part, throughout the world, in perpetuity, in any media now known or hereafter developed.
- This release to TCS includes, the right to use the Testimonials either alone or together with supporting information, such as your name, voice, photograph, likeness and biographical data (collectively, "Supporting Information").
- Personal information included in the recording of the Testimonials, together with the supporting information and the other details however collected with this form, will be processed by TCS according to the TCS Privacy Notice (https://www.tcs.com/who-we-are/legal/tcs-privacy-notice) together with this form and any other specific documentation that may be provided to you at the time you give your consent to such processing.
- With this consent, you're authorizing the usage of the Testimonials for purposes including but not limited to:
- Testimonial Quotes - a brief, personalized statement describing your experience, to be used in corporate brochures, corporate website, social media channels and corporate Testimonials.
- Written and Video Case Study – a document/story describing your experience with TCS to be used in sales collateral, recruiting campaigns, corporate Testimonials and corporate website and/or a recording of your speech and any post speech comments to be used in sales collateral, recruiting campaigns, corporate Testimonials and corporate website.
- Testimonial Advertisement – an advertisement describing your experience with TCS
- You can request TCS to stop using the Testimonials at any time, in which case the Company will not use the Testimonials for new purposes as from that moment and will attempt to remove where feasible, but it may continue to appear in publications already in circulation.
- TCS will continue using the Recorded Material unless you request us to not actively use it anymore. You can make the request at any point in time by contacting our Data Protection Officer at dpo.uki@tcs.com (for UK and Ireland) or dpo.europe@tcs.com (for other areas of the EEA).
- TCS will ensure by all means possible to not make use of the Testimonials in a way that is defamatory, unlawful, detrimental for or in violation of your rights.
I confirm that:
- I understand that I am solely responsible for all content and material provided within my Testimonials. I will ensure that all third-party material used by me (including but not limited to presentations, audio and video material, images and brand names) is covered by a permission, release or license to enable the recording of my Testimonials consistent with the possible usages set at articles 1 and 4 above.
- I ensure to TCS that the content of the Testimonials is not defamatory, unlawful and, to the best of my knowledge, does not violate the rights of any third party.´
- I release TCS from any claims or demands arising from the use of the Testimonials. TCS will not be responsible for any unauthorized use of my Testimonials.

- With the participation to the event and the acceptance of this consent, you grant TCS, its affiliates and other entities that TCS may designate from time to time, the right to record, stream, film and photograph your presence and activities at the Event (the "Testimonials") and to distribute, broadcast, stream, edit, translate or otherwise disseminate it, without any further approval from you, in whole or in part, throughout the world, in perpetuity, in any media now known or hereafter developed.
- This release to TCS includes, the right to use the Testimonials either alone or together with supporting information, such as your name, voice, photograph, likeness and biographical data (collectively, "Supporting Information").
- Personal information included in the recording of the Testimonials, together with the Supporting Information and the other details however collected with this form, will be processed by TCS according to the attached Annex 1 - Privacy Notice and together with the one provided to you during the onboarding process, sometimes referred to as Employee Privacy Notice or GDPR Employee Privacy Notice (altogether the "Privacy Notice").
- With this consent, you're authorizing the usage of the Testimonials for purposes to:
- Testimonial Quotes - a brief, personalized statement describing your experience of working for TCS, to be used in corporate brochures, corporate website, social media channels and corporate Testimonials.
- Written and Video Case Study – a document/story describing your experience of working for TCS to be used in sales collateral, recruiting campaigns, corporate Testimonials and corporate website and/or a recording of your speech and any post speech comments to be used in sales collateral, recruiting campaigns, corporate Testimonials and corporate website.
- Press Release – issued to press and media in order to make a newsworthy announcement relating to attendance at the event.
- Testimonial Advertisement – an advertisement describing your experience of working for TCS.
- You can request TCS to stop using the Testimonials at any time, in which case the Company will not use the Testimonials for new purposes as from that moment and will attempt to remove where feasible, but it may continue to appear in publications already in circulation.
- At the moment of the termination of your employment relationship with for any reason, TCS will keep on using the Recorded Material as if the employment relationship was not ended, unless you request not to actively use it anymore. You can make the request at any point in time.
- TCS will ensure by all means possible to not make use of the Testimonials in a way that is defamatory, unlawful, detrimental for or in violation of your rights.
- For any queries about your data processed under this consent form or to exercise your data protection rights, please contact our Data Protection Officer at dpo.uki@tcs.com (for UK and Ireland) or dpo.europe@tcs.com (for other areas of the EEA).
- Sufficient compensation/remuneration for using the Testimonials is deemed to be included in the salary agreed in the employment relationship and therefore you will not be entitled to extra compensation/remuneration in any form.

Privacy Notice
This privacy notice describes how Tata Consultancy Services Switzerland Ltd., Zurich collects and use personal data about you in connection with the above consent in accordance with the General Data Protection Regulation (GDPR) and the applicable data protection legislation.
This notice applies to all the TCS employees who agreed to the above consent form.
It is important that you read this notice, together with any other privacy notice we may provide on specific occasions when we are collecting or processing personal data about you, so that you are aware of how and why we are using such information. In particular, it is necessary for you to read the Employee Privacy Notice provided by TCS to you at the moment of your onboard and available at every moment in the TCS Ultimatix platform through the “Notices & Consents” application (Ultimatix -> Security, Risk, Compliance & Privacy -> Notices & Consents); the Employee Privacy Notice applies to the current processing with the addition of what stated into this document.
THE KIND OF INFORMATION WE HOLD ABOUT YOU
Personal data means any information about an individual from which that person can be identified. It does not include data where the identity has been removed (aka anonymous data).
In addition to the information already processed by TCS by the employment relationship, we will also collect, store and use your digital images, video and audio recordings .
HOW IS YOUR PERSONAL DATA COLLECTED
We collect personal data about you through recordings of the Event and/or verbally, either directly from you or from TCS internal database.
HOW WE WILL USE INFORMATION ABOUT YOU
We will only use your personal data when the law allows us to. Most commonly, we will use your personal data in the following circumstances:
- Where you have consented to such use.
- Where we need to comply with a legal obligation.
- Where it is necessary for our legitimate interests (or those of a third party) and your interests and fundamental rights do not override those interests.
Situations in which we will use your personal data
We also process personal information for certain legitimate business purposes, which include some or all of the following:
- Digital images including photographs and video, according to the terms set out in the Employee image consent form.
- To administer and fulfill any other purpose and obligation set out in the consent.
- Defend TCS for any legal claims or investigations.
Some of the above grounds for processing will overlap and there may be several grounds, which justify our use of your personal data.
Whenever we process data for these purposes we will ensure that we always keep your Personal Data rights in high regard and take account of these rights.
DATA SHARING
We may have to share your data with third parties, including third-party service providers and other entities in the group. Other than and in addition to the third parties identified in the Employee Privacy Notice, we will share data with the following subjects.
